If you wish to have a fundraiser or event added to the calendar please click on the link below, complete the form and bring it to the parish office for approval and reservation of the date.


Decree on Child Protection

Employees and volunteers 18 years or older who are involved with programs involving children at our parish are required to register for a SafeParish account and complete the background and training requirements. Registration instructions are below. Please contact our local safe environment coordinator, Chad Riegert at criegert@qphamilton.org, with questions.

SafeParish Registration Instructions

  1. Go to www.AoCSafeEnvironment.org.
  2. Click "Register" on the top right of the home page
  3. Enter the passcode protectcincy when prompted.
  4. Select the location from the drop down where you are employed or volunteer.
  5. Click the role(s) associated with your employment/service at the parish, school, or Archdiocesan institution. Select a sub-role from the drop down to describe your employment/service at this location.
    If you are active at only this location, this will be your primary location.
    If you are active at more than one location, click the ADD PARISH/LOCATION button and repeat the steps above. With more than one location, please click the "This is my main/primary location" radio button to indicate where you are employed or, if you are a volunteer, where most of your ministry or service is performed.
  6. Complete your personal information. Please enter your legal name for the purposes of the background screening. There are no credit checks and no soft hits to credit.
  7. Create a username, password and password clue.
  8. If information regarding your previous safe environment account can be located, it will be displayed. Click the radio button for the correct choice or choose none of the above if none of the information pertains to you. If your information was found and you clicked the appropriate radio button information regarding your training and background check dates will be transferred into your SafeParish account.
  9. Agree to the Terms of Use and click SUBMIT.
    If you have not previously completed an online background check you will be prompted to submit information for a background screening through SELECTION.com.
    You will be required to acknowledge that you have downloaded and accepted the terms of the Decree.
    If you have not previously completed child protection training you will be prompted to view SafeParish: Protecting Children From Sexual Abuse — Archdiocese of Cincinnati. (If you have completed a background check through VIRTUS, no new background check will be required also if you have attended VIRTUS training, no new training will be required.)
    If you have completed the required background checks and child protect training, you will be prompted to click the link to Continue to the Home Page. Your registration is complete.
    If you need to complete background check requirements or training, you may use the links in your approval checklist to access these requirements. These steps may be completed all at once or may be completed separately at your convenience. You will receive an automated email weekly to remind you which requirements are outstanding. When all compliance requirements have been completed, your account will be approved, and you will be clear to work/volunteer with youth and vulnerable adults.

The Archdiocese of Cincinnati regards child abuse in all its forms as a serious matter. Anyone who knows of or suspects child abuse by a priest, employee, or volunteer of the Archdiocese should first report the abuse to the appropriate secular legal authority and then contact the Coordinator of Ministry to Survivors of Abuse of the Archdiocese at 513.263.6623 or 1.800.686.2724 ext. 6623.


Facility User Agreement for Parishioners

If you wish to use any of the facilities on the parish campus for your event other than a parish event you must complete the Facility Use Agreement for for Parishioners and get approval. Please click on the link below, complete the form and return it to the parish office.


Funeral Planning Guide

These documents contain the funeral planning worksheets along with all of the readings, petitions, and music options a family will need in order to plan the funeral Mass of a loved one. This is the same worksheet that will be completed with a member of the berevement committee when meeting at QP to plan a funeral Mass.

The Holy Family Thanksgiving & Memorial Garden

This form is to be used when wishing to order bricks for The Holy Family & Memorial Garden. This is the landscaped area around the holy family statue (in front of the old church) which has been made into a "Thanksgiving & Memorial Garden" for the parish. Through the placement of an engraved brick in the garden walkways, parishioners and friends can establish a lasting memorial on the parish grounds to a deceased friend or loved one or a perpetual sign of their thanks to God for a special blessing He had bestowed upon them, their family, or friends.

New Parishioner Registration Form

Registration Forms are available online and in the parish office Monday - Friday. Please notify the parish office if your name, address, telephone number or marital status has changed. Current parishioners who have plans to move from the parish should also notify the parish office.
